Betty Anne Farrell was a founding Board member of the Collaborative Center whose life touched many people in South Boston.  To celebrate her life and continue her desire to help others, the Farrell's have graciously decided to institute an annual award of a cash donation to be given to a single mother from South Boston.

     The award is a cash grant of one thousand dollars, which will be presented to the recipient or her designee at the Collaborative fundraiser on January 26, 2006.  The below listed criteria are guidelines for those wishing to nominate someone. 

• The person being nominated is a single mother from South Boston.
• Those wishing to nominate someone should write a letter explaining why they consider this person a candidate for this Award.  The nomination letter limit is one page.  The nomination letter should clearly state your reasons for the nomination, length of time you have know the person and what actions demonstrate this person’s commitment to positive change.
• The person being nominated does not have to be a person from the recovery community.
• The recipient of the Award does not need to attend the presentation ceremony; however a representative from your agency is requested to be present to receive the Award on the recipient’s behalf.
• The deadline for nominations is December 23, 2005 at 5pm.  All nominations should be sent to the Collaborative Center, 8 Rev. Burke Street, Apt 151, South Boston, MA, 02127.  We will forward them to the Farrell family for review.  The family will be deciding the Award recipient.
